INFORMED CONSENT FORM

Drop-Down List Boxes vs. Standard List Boxes For Varying Numbers of List Items

This is to certify that I, __________________, hereby agree to participate as a volunteer in a scientific investigation as an authorized part of the education and research program of the Department of Computer Science of the University of Maryland, College Park, under the supervision of ___________________, a researcher. I am over 18 years of age and have experience with the Windows 95 user interface.

I have been given an opportunity to ask any questions I may have had regarding this experiment, and all such questions and inquiries have been answered to my satisfaction. I understand that I am free to deny any answers to specific items or questions in interviews or questionnaires. I understand that any data or answers to questions will remain confidential with regard to my identity.

I understand that the purpose of this experiment is to investigate information presentation in dialog boxes.

I further understand that I am free to withdraw my consent and terminate my participation at any time.
